Ohio ID# OH3 800 015 379. You may submit written comments to Don Vogel, Ohio EPA-DERR, P.O. Box 1049, Columbus, Ohio 43216-1049. All written comments must be received before the close of business on May 18, 2015. The closure plan is available for review at Ohio EPA Central Office, Division of Environmental Response and Revitalization, 50 West Town Street, Suite 700, Columbus, Ohio 43215, Tel: (614) 644-2924. The closure plan is also available for review at Ohio EPA Northwest District Office, 347 North Dunbridge Road, Bowling Green, Ohio 43402, Tel: (419) 352-8461.
